Please complete this reservation form as completely as possible and submit it to us by clicking on the submit button on the bottom of this page.

Be sure to fill out your name, address, e-mail address, and dates you would like to dive with us. We also need to know how many dive trips you are thinking about, how many in your party are diving with us, and if you need to rent any equipment.

We will use the information you submit to provide you with a quote via return e-mail for the dive trips and rental equipment you request.

In order to reserve your diving with Blue Magic Scuba we ask that you provide us with a deposit (the amount of the deposit will be detailed in our return e-mail correspondence with you).
